La chirurgie de l'asymétrie

Résumé

La chirurgie de l'asymétrie constitue un chapitre du rapport de la 75e réunion scientifique de la Société Française d'Orthopédie Dento-Faciale.

Il est utopique de vouloir présenter, en quelques pages, un travail exhaustif de toutes les propositions chirurgicales, en regard de la multitude des étiopathogénies.

L'auteur envisage l'évaluation du patient asymétrique et souligne, dans les examens complémentaires, l'intérêt de la tomodensitométrie tridimensionnelle. Un chapitre est réservé à la préparation orthodontique préchirurgicale, visant à supprimer les compensations qui affectent le support alvéolaire maxillaire et mandibulaire. La stratégie thérapeutique fait appel à des programmes «combinés». Une «palette» des ostéotomies est proposée dans le cadre des dysmorphies courantes. Deux pathologies plus complexes, que l'orthodontiste peut rencontrer, sont évoquées : l'hypercondylie et l'ankylose temporo-mandibulaire liée aux affections inflammatoires et aux traumatismes. La glossoplastie est de règle dans certaines hémihypertrophies.

Les récidives, qui entachent la stabilité du résultat, peuvent être contrecarrées par des surcorrections, certains moyens de fixation et une kinésithérapie post-opératoire.

Une iconographie et des observations cliniques illustrent la démarche de l'orthodontiste comme celle du chirurgien : elles sensibilisent le lecteur aux différents problèmes liés à l'asymétrie.

Abstract

The surgery of asymmetry constitutes a chapter of the report of the 75th scientific meeting of the "Société Française d'Orthopédie Dento-Faciale" (French Society of Dentofacial Orthopedics).

Endeavoring to present, within a few pages, an exhaustive work of all the surgical proposals, regarding the multitude of ethiopathogenies, is quite utopic.

The author considers the evaluation of the asymmetrical patient and underlines in the complementary examinations the advantage of 3D tomodensitometry. A chapter is devoted to presurgical orthodontic preparation aimed at counteracting the compensations, which affect the maxillary and mandibular alveolar support. The therapeutic strategy calls upon "combined" programs. A "range" of osteotomies is suggested within the framework of the most frequent dysmorphoses. Two more complex pathologies, which may be met by the orthodontist, are evoked: condylar hyperplasia and temporomandibular ankylosis related to inflammatory affections and traumatisms. Glossoplasty is the rule in some hemihypertrophies.

Relapses, hindering the stability of the result, can be thwarted by overcorrections, different means of splinting and postoperative kinesitherapy.

Iconography and clinical observations illustrate both the orthodontist's and the surgeon's approach: they heighten the reader's awareness of the various problems linked with asymmetry.
